2026 Fall Sports Registration
Angels fly higher!
Parent/Guardian must fill out the entire registration form to sign up for Fall 2026 Sports Teams. No student will be permitted to tryout without clearance from the Health Office.
You will need to UPLOAD to Magnus:
- NJ Pre-Approved Physical Form (if not already uploaded in summer).
- Health History Update Form (if 90 days have passed since your last physical)
- Immunization Record must be uploaded as well (if not already uploaded in summer).

Faxed physicals WILL NOT BE ACCEPTED- it must be uploaded to your Magnus Health Portal.
PLEASE DO NOT MAIL PHYSICALS
Physicals are not accepted by physicians who are not licensed to practice in the US.
Students with asthma and/or allergies: Asthma/Epi-Pen Forms, also on your Magnus Health Portal, MUST BE INCLUDED WITH THE PHYSICAL.
