AVON GROVE MIDDLE SCHOOL

Parents/Guardians,

PHYSICALS MUST BE DATED May 1, 2025 OR AFTER, to be valid for the 2025-2026 school year. They are then valid until May 31st, 2026 or the conclusion of the spring sports season.

If this is the first sport you are participating in at Avon Grove School District:

Please click "Sign Up" (in the upper right corner) and create a profile.  Log in to FormReLeaf.  Go to "Find Organization" and type in Avon Grove School District.  Click on "Avon Grove School District” then scroll down to “Programs” and click on “2025-2026 Winter Sports - *Your School*”.   Choose a sport and scroll down to “Participant Information”.  Fill in the information for the student-athlete for whom you are registering.  **If you have used FormRELeaf in the past, you can autofill your information from a previous registration as long as you are signed into your account.**  Once you have filled in the information, “Submit” will appear at the bottom.  Click the "Submit" button and your student-athlete will be registered for their winter sport.  You will receive a confirmation email. IF YOU DO NOT RECEIVE AN EMAIL, IT DID NOT GO THROUGH, REVIEW THE REGISTRATION AND COMPLETE THE MISSING INFORMATION AND RESUBMIT IT!

Section 6:

If you have not already done so, please print out section 6 of the PIAA CIPPE form here.  This is the physical page and needs to be completed, signed and dated by a physician.  It must be dated May 1, 2025 or later to be accepted for the 2025-2026 school year.  Once completed, it should be uploaded under the Physician section.  If you are unable to scan and submit it, the form may be submitted to the athletics office at the high school.  The athletic training staff will verify all registrations once submitted.  Once reviewed, including the physical, the registration will be approved, and another email notification will be sent from FormReLeaf.  If there is an issue, FormReLeaf will send an email notification of the issue.  If you have not submitted a physical, you will not receive an approved notification for the registration.
