Calvary Christian Academy

Athletic Clearance for the 2022-2023 school year will be available July 1st. No documents will be accepted before this date. 

Prior to beginning the clearance process, you must have the following information completed in digital form:

1. ​An FHSAA EL2 - Preparticipation Physical Form Signed and Dated by all Parties, including a Licensed Physician
2. Copy of Insurance Card -OR- a CCA student accident insurance application (additional cost)
