2026–27 Athletic Participation Requirement

To help streamline the paperwork process before tryouts, we have partnered with FormReLeaf. Every student who plans to tryout for a sport must register through the FormReLeaf system.

Once you register, your information will remain in the system. For example, if you participate in a fall sport and would like to play a spring sport, you will simply need to review and update your information at that time.

Before Tryouts, Every Student Must:

• Register in the FormReLeaf system.
• Have an up-to-date physical on file with the School Nurse.

Please note that all students must have a current physical on file in the Nurse’s Office, regardless of their FormReLeaf registration.
