Winter Sports 2019-20

PLEASE BE SURE THAT YOUR CURRENT PHYSICAL IS ON FILE WITH THE SCHOOL NURSE. ALSO, HAVE YOUR MEDICAL, INSURANCE INFORMATION AND SCHOOL ID NUMBER HANDY, PRIOR TO STARTING THIS REGISTRATION.

       or  completed "Sports Physical" is due to School Nurse by November 1st, 2019.  

                  This is then taken to the School Physician for final approval before allowed to participate   

If the student's Physician did the Exam more than 90 days ago;

                  You will need to complete a Health History Update Questionnaire and return to the School Nurse by November 1st.  

                                     NOTE:  A Health History Update Questionnaire MUST BE COMPLETED prior to a each season a student-athlete competes.

***Costs/Sport are estimates and may change based on the number of participants in each sport. The final Cost/Athlete will be communicated to you at the start of the season.***
