2020-21 Sports and Health Office Registration

Welcome to CVU's online sports and health office registration platform. The information requested below is REQUIRED information for ALL CVU students whether they are playing a sport or not. By providing this information, the nurses in the health office will have this important, safety information and your child will be cleared for all school related field trips. You can also register your child for multiple sports at the same time i.e. you fill out, or confirm, the information once!

IF you completed the on-line registration last year simply use the SAME email address and log-in. You MUST select "All Students-Emergency Medical Information" AND IF your child is anticipating playing a sport, select the appropriate season (fall, winter, and/or spring), gender (if applicable) and sport e.g. fall-girls-soccer or winter-boys-hockey.  If your child is NOT playing a sport at CVU this year, simply confirm the information provided is correct and submit. Please make sure to change any essential information e.g. phone numbers or medical/insurance information before submitting. At a minimum, please change your child's GRADE in school. Any items marked with an "*" (asterisk/red star) are "required" items and your registration will not be accepted if that item is not answered. To submit, click the "Submit" button located in the lower right-hand corner.

IF this is your first time registering a student using Form ReLeaf, you will need to complete the information requested below.   The process will take 15-20 minutes and you will need to have/know your doctor's phone number, insurance information, etc. Any items marked with an "*" (asterisk/red star) are "required" items and your registration will not be accepted if that item is not answered.

When you have successfully completed the online registration process, you will receive an almost instantaneous confirmation email.

We anticipate that sports will be allowed to take place this year. As a result of COVID, the following changes are being made for THIS year.  IF we have your child's current physical and it is set to expire during THIS school year, that physical is extended for ONE year. e.g your child's physical is set to expire on October 1, 2020...it is now good until Oct 1, 2021. 

HOWEVER, if we do NOT have a current physical for your child, s/he MUST obtain a sports physical (link below) before they will be allowed to tryout/practice or play this year. In addition, athletes are required to have a current physical.  The main office must have a current (dated within two years) "CVU Athletics Well-Exam" on file before the athlete may try-out, start practice, or participate in our co-curricular programs. If you have not filed the "Athletics Well Exam" form with us, please "click" on the link below,  print out the form and bring it to your physician to complete. Once completed, you can:

• Bring it to us;

• Mail it (369 CVU Road, Hinesburg VT 05461); 

• Fax it (802.482.7108) to the main office; OR

• Scan and email it to: dshepardson@cvsdvt.org

We MUST have the physical form on file prior to the start of your child's season.  Fall sports are scheduled to begin on Wednesday, August 26. Winter sports start on Monday, November 30 and Spring sports start on Monday, March 22, 2021.
