2018/2019 ATHLETE REGISTRATION (MS)

You must be eligible to participate in Middle School Sports. Please review the following list with your parents. If you have questions, see your coach or athletic director. You must complete this registration before tryouts.

Steps for being approved to participate:

1. Get an up to date Sports Physical
2. Complete all registration items and upload a scanned copy of your physical to formreleaf.com
3. Await approval status on formreleaf. (You should receive an email notification. You may need to check your spam folder if you don't see it in your inbox.)
4. Status must be Approved (Once everything is completed the athletic director reviews everything and gives the final approval. This cannot be done by a coach)

STUDENT ELIGIBILITY

Students in grades 7th and 8th are required to have a 2.0 from the previous grading period.

Students in the 6th grade are automatically eligible for fall sports. 

They must have a 2.0 from the previous grading period to participate in winter and spring sports.

-Upload a copy of your most recent physical or turn it into your Athletic Director. 

The physical exam must be completed by a licensed physician, certified physician’s assistant under the supervision of a licensed physician or a certified nurse practitioner.
