Middle School Intramural - Interscholastic Program Overview

• This is the online registration information for Clifton Middle School Athletics. Registration for all interscholastic sports must be completed online through FormReLeaf and submitted by the registration deadline.

  Requirements for Participation in Middle School Athletics

  Students are eligible to participate ONLY after all required registration and medical documentation has been completed:

  1. A Parent/Guardian must complete the Athletic Registration Form through FormReLeaf by the posted deadline.
  2. A current Preparticipation Physical Evaluation (PPE) must be completed by a physician and submitted to the Site Director or School Nurse at the student’s respective middle school.
  3. All required forms and medical clearances must be approved before a student may participate in practices or tryouts.

  Please note: ALL requirements must be met before a student will be cleared to participate.

  Physical Examinations

  Students participating in middle school athletics must have a current physical examination on file before participating in any practice or tryout.

  Physical examinations are valid for 12 months from the date of the examination.

  Completed physical forms should be submitted directly to the Site Director or School Nurse at the student’s respective middle school.

  Middle School Athletic Programs

  Intramural Sports – Recreational athletic activities conducted within the school and designed to provide students with opportunities for participation, physical activity, and skill development.

  Interscholastic Sports – School-sponsored athletic teams that compete against teams from other schools.
