2022-2023 Winter Sports - 7TH AND 8TH GRADE

Parents/Guardians,

PHYSICALS MUST BE DATED JUNE 1, 2022, OR AFTER, to be valid for the 2022-2023 school year. Once uploaded physicals are valid until May 31st, 2023 or the conclusion of the spring sports season.

If this is the first sport you are participating in at the high school:

IF YOU DO NOT RECEIVE AN EMAIL, IT DID NOT GO THROUGH AND YOU MUST TRY AGAIN!

If you have not already done so please print out section 6 of the PIAA form here.  This is the physical page and needs to be completed and signed by a physician.  It must be dated June 1, 2022 or later to be accepted for the 2022-2023 school year.  Once completed it should be uploaded under the Physician section.  If you are unable to scan and submit it, the form may be submitted to the Head Athletic Trainer.
