Welcome to CHS's online sports and health office registration platform. The information requested below is REQUIRED information for ALL CHS students if they want to participate in a sport. By providing this information, the nurses in the health office will have this important, safety information and your child will be cleared for all school related field trips.

When you have successfully completed the online registration process, you will receive a confirmation email.

In addition, athletes are required to have a current physical. The main office must have a current (dated within two years) "Well-Exam" on file before the athlete may try-out, start practice, or participate in our co-curricular programs. If you have not filed the "Well Exam" form with us, please click on the 2 links below to complete.

Once completed, you can:

Have your student hand it in to the Athletic department;

Mail it (CHS, PO Box 900, Colchester VT 05446);

Fax it (802.264.5757) to the main office; OR

Scan and email it to: phyllis.etienne@colchestersd.org

We MUST have the physical form on file prior to the start of your child's season.
