Form submission deadlines: Fall 8/1/19, Winter 11/1/19, Spring 3/1/20

First sport/sport season during the 2019-2020 school year:

Step 1: Complete the FormReLeaf registration below.

Step 2: Complete & return the NJ Preparticipation Physical Evaluationl (PPE)

  • Page 1 must be completed by the parent.  
  • Page 3 & 4 must be completed by a private physician.
  • If the student requires emergency medication (Epipen or inhaler) forms completed by the private physician are required.  Permission for self-administration is strongly encouraged.  If self-administration permission is not included, 2 sets of EpiPens are required - One to remain at school and one for the Coach to carry.   Medication Forms
  • NJDOE Scholastic Student Athlete Safety Act states that prior to participation on a school-sponsored interscholastic athletic team or squad, each student athlete in grades six through 12 must present a completed PPE form.  The required PPE must be conducted within 365 days prior to the first day of tryouts. 
  • The Board of Education Physician must approve the PPE performed by a private doctor after it is submitted to the Health Office before the student will receive final clearance to participate.  This process that can take up to 10 days.

Step 3: Athletes participating in at least one sport and any activities must pay the 2019-2020 activity fee of $100.00 before try outs begin via PayForIt.  Students participating in activities but NO sports must pay a fee of $75.00.  The maximum fee for a family (all schools) is $350. 

  • To register on the PayForIt website the PowerSchool student ID number following the Last name, First name, MI on the Demographic Update page in the parent portal of PowerSchool,  is necessary.

Sometimes step 4: Complete the Health History Update Questionnaire and return it to the health office ONLY if it has been more than 90 days since the physical exam.

Second and/or third sport/sport season during the 2019-2020 school year:

1 Step: Complete the New Jersey Health History Update Questionnaire and return it to the health office.

  • This form must be completed by a parent and is required when it has been more than 90 days since the student had a sports physical as is the case when the physical was given by the school doctor in June 2018 and/or it is the second or third sport that a student is trying out for during the 2019-2020 school year.
  • Unless the student athlete’s physical was conducted more than 365 day prior to the first official tryout in the athletic season. In that case a new physical and a new NJ Preparticipation Physical Evaluation (PPE) is required before the form submission deadline.
