To Dolgeville Athletes and their Families:

In an attempt to streamline our paperwork process to play a sport, we have partnered with Form RE Leaf. Each student looking to play a sport must register in this system.

Once you have registered, you will become a part of the system. For example, if you play a Fall sport and want to play another in the Spring, you will only need to update your info at that time.

All Dolgeville students still must have an up-to-date Physical on file in the Nurse's Office. 

Here are the two items a student must have prior to the first day of practice...

1 - register in the Form RE Leaf system.

2 - provide the School Nurse an up-to-date Physical.
