2021-22 Winter Sports Registration (Basketball)

Hello Parents & Student-Athletes and Welcome to Form Releaf, 

Form Releaf is a secure registration platform that provides you with an easy, user-friendly way to register for Dundalk Middle School Athletics. Winter  Sports registration will be done electronically except for the sports physical forms and the Covid-19 sign-off sheet. Sports physical forms can be scanned and sent to Athletic Department.

To Register: 
1. Select the Season, Select the Sport, Fill in all the fields, and Review Heat Acclimatization, Sudden Cardiac Awareness, Informed Consent Video, Parent/Athlete Contract, Code of Conduct, and Concussion Policies. Sign off using an Electronic Signature when you have completed the online registration process.

ACADEMIC ELIGIBILITY FOR BASKETBALL WILL BE BASED OFF OF 1ST QUARTER GRADES.  STUDENTS MUST RECEIVE AT LEAST A 2.0 GPA WITH NO MORE THAN 1 FAILING GRADE DURING THE 1ST MARKING PERIOD IN ORDER TO BE ACADEMICALLY ELIGIBLE FOR THE WINTER SEASON TRYOUTS.  STUDENTS MUST MAINTAIN THOSE ACADEMIC REQUIREMENTS THROUGH THE SEASON (2ND QUARTER) IN ORDER TO FINISH THE SEASON IN GOOD STANDING.
