Hello Parents & Student-Athletes and Welcome to Form Releaf,

Form Releaf is a secure registration platform that provides you with an easy, user-friendly way to register for Dundalk Middle School Athletics. Fall Sports registration will be done electronically except for the sports physical forms and the Covid-19 sign-off sheet. Sports physical forms can be scanned and sent to the athletic office.

To Register:
1. Select the Season, Select the Sport, Fill in all the fields, and Review Heat Acclimatization, Sudden Cardiac Awareness, Informed Consent Video, Parent/Athlete Contract, Code of Conduct, and Concussion Policies. Sign off using an Electronic Signature when you have completed the online registration process.

2. Physical Form with the Covid-19 Update-https://www.mpssaa.org/assets/1/6/Physical_Evaluation_Form.pdf
