2021-2022 Fall Sports Registration

FALL 2021 REGISTRATION AND PHYSICALS DUE BY AUGUST 6TH

Hello Parents & Student-Athletes and Welcome to Form^RELeaf, 

Form^RELeaf is a secure registration platform that provides you with an easy, user-friendly way to register for Eastern Technical High School Athletics. All Fall 2021 Sports registrations will be done electronically. Physical forms can be uploaded directly to the Form^RELeaf website.

Physicals are valid for fourteen (14) months from the date of the exam and must be on the approved form (Required MPSSAA Physical Form). Student-athletes who do not have a valid sports physical and do not register will not be permitted to try out for fall sports. No exceptions.

To Register: 

1.      Select the Season, the Sport, and complete all the required fields (including uploading a valid physical on the MPSSAA form).

2.      Review the Concussion Information Sheet, Student-Athlete Informed Consent Video, Sudden Cardiac Arrest (SCA) Policy, Heat Acclimatization Guidelines for Student-Athletes, Athletic Department Handbook, COVID-19 Information, and the BCPS Athletic Permit.

3.      Sign the Acknowledgement Statement using an electronic signature after you have completed the online registration and reviewed all the listed items.
