2024 Spring Sports Registration

Spring 2024 REGISTRATION AND PHYSICALS DUE BY FEBRUARY 22nd

Hello Parents & Student-Athletes and Welcome to Form Releaf, 

Form Releaf is a secure registration platform that provides you with an easy, user-friendly way to register for Eastern Technical High School Athletics. 20243 Spring Sports registration will be done electronically. Physical forms can be uploaded directly to the Form Releaf website. If you have trouble with the upload, please scan and email the document to our Athletic Trainer- Ms. Magladry at amagladry@bcps.org  (PDF File)

Physicals are valid for 14 months from the date of the exam and must be on the approved form (link for form below). Student-athletes who do not have a valid sports physical and do not register will not be permitted to tryout for spring sports. No exceptions. Any physicals received after February 22nd may delay participation in tryouts.

To Register: 

1. Select the Season, Select the Sport, Fill in all the fields, and Review Heat Acclimatization, Sudden Cardiac Arrest Policy, Informed Consent Video, Concussion Information Sheet, COVID-19 Information, BCPS Athletic Permit, ETHS Athletic Department Handbook, and the ETHS Athletics' Information Padlet. Sign off using an Electronic Signature when you have completed the online registration process.  

2. Physical Form with the Covid-19 Update-mpssaa.org
