Fall Sports Registration Will Open on 7/1/26

Deadline to register and submit all medical forms is 8/19/26

Summer Hours for Submitting Medical Eligibility Forms or HHUQ

We will collect the Medical Eligibility Forms or Health History Update Questionnaires on Tuesdays and Thursdays from 10:00 am to 12:00 pm at the security desk at Morristown High School

Health History Update Questionnaires are required for any physical older than 90 days. 

Frelinghuysen Middle School offers the following sports;

 Fall- Boys Soccer **, Cross Country, Girls Soccer**, Field Hockey

Winter- Boys Basketball **, Cheerleading**, Girls Basketball**, Wrestling

Spring- Baseball**, Boys Lacrosse**, Girls Lacrosse**, Softball**, Track

** Cut Sports**


       The registration process can take 2 weeks from the time you upload paperwork until the time you are cleared.

Step 1: Health Forms

If you HAVE a valid physical recorded in the FMS health office dated 9/2/25 or after, complete these forms:

• Health History Update Questionnaire (must be completed every season) Health History Update
• Opioid Form (must be completed every season)
•  If you DO NOT have a current physical on file dated 9/2/25 or after, complete these forms and upload: NOTE- These forms were updated by NJ as of 9/1/24
• All forms in the PACKET and Opioid Form- hard copies must be submitted directly to health office

Note:

Step 2: Online Registration 

You must complete the online registration for every sport each season. Student ID number is required for online registration. 

The athletic registration needs to be completed by a parent/guardian AND student together.

Returning User:New User:  Please log in as a "new user-create an account" and complete the form.  

Please make sure at the end of the process you create a username and password this is the final step in the registration.  

If the final step is missed your registration will not be processed.
