2023 Fall Athletic Registration

Hello Parents & Student-Athletes and Welcome to Form Releaf,
Form Releaf is a secure registration platform that provides you with an easy, user-friendly way to register for Golden Ring Middle School Athletics. Fall Sports registration will be done electronically except for the sports physical forms. Sports physical forms can be scanned and uploaded in this website but paper copies may be requested if the upload is not legible.
To Register:
1. Select the Season, Select the Sport, Fill in all the fields, and Review Heat Acclimatization, Sudden Cardiac Awareness, Informed Consent Video, Parent/Athlete Contract, Code of Conduct, and Concussion Policies. Sign off using an Electronic Signature when you have completed the online registration process.
2. Physical Form with the Covid-19 Update (this page is no longer necessary in Spring of 2023) -https://www.mpssaa.org/assets/1/6/Physical_Evaluation_Form.pdf

Registration for this season will close on Friday September 8, 2023* this has been extended to September 15
Important Dates for Fall Sports 2023:
Fall practices will be held after school until 4:30pm on the following days Badminton= Tues-Fri, Cross Country= Mon-Thurs and Allied Soccer on Wednesdays and Fridays.
Parents are responsible for student athlete pick up.
Each fall sport will have a tournament and championship on two separate Saturdays during the season.
DATES FOR FALL SPORTS COMPETITIONS INCLUDE:
September 30th=All fall sports will all have their first competitions- locations listed below:
Allied soccer and badminton meets at Loch Raven Technical Academy
Cross country meet at Perry Hall Middle School
October 14th= Allied soccer will have the culminating event at Perry Hall High School
October 21st= Badminton will have their championship at Dundalk High School
October 21st= Cross Country will have their championship meet at Loch Raven High School
Dates and times are subject to change- there is no rain date at this time
