GREAT VALLEY HIGH SCHOOL

Parents/Guardians,

PHYSICALS MUST BE DATED JUNE 1, 2022, OR AFTER, to be valid for the 2022-2023 school year. Once uploaded physicals are valid until May 31st, 2023 or the conclusion of the spring sports season.

If this is the first sport you are participating in at the high school:

Please "Sign Up" (in the upper right corner) and create a profile.  Log in to FormReLeaf.  Go to "Find Organization" and type in Great Valley High School.  Click on "Great Valley High School” then scroll down to “Programs” and click on “2023 Spring Sports”.   Choose a sport and scroll down to “Participant Information” and fill in the info for the student-athlete for whom you are registering. Once you have done that, “Submit” will appear at the bottom and after you click that, your student-athlete will be registered for their fall sport.  You will receive a confirmation email. IF YOU DO NOT RECEIVE AN EMAIL, IT DID NOT GO THROUGH AND YOU MUST TRY AGAIN!

If you have not already done so please print out section 7 of the PIAA form here.  This is the physical page and needs to be completed and signed by a physician.  It must be dated June 1, 2022 or later to be accepted for the 2022-2023 school year.  Once completed it should be uploaded under the Physician section.  If you are unable to scan and submit it, the form may be submitted to the Head Athletic Trainer.
