GREAT VALLEY HIGH SCHOOL

Parents/Guardians,

PHYSICALS MUST BE DATED MAY 1, 2024, OR AFTER, to be valid for the 2024-2025 school year. Once uploaded physicals are valid until April 30th, 2025, or the conclusion of the spring sports season.

If this is the first sport you are participating in at the high school:

Please "Sign Up" (in the upper right corner) and create a profile.  Log in to FormReLeaf.  Go to "Find Organization" and type in Great Valley High School.  Click on "Great Valley High School” then scroll down to “Programs” and click on “2024-2025 Winter Sports”.  Choose a sport and scroll down to “Participant Information” and fill in the info for the student-athlete for whom you are registering. Once you have done that, “Submit” will appear at the bottom and after you click that, your student-athlete will be registered for their winter sport.  You will receive a confirmation email. IF YOU DO NOT RECEIVE AN EMAIL, IT DID NOT GO THROUGH, REVIEW THE REGISTRATION AND COMPLETE THE MISSING INFORMATION AND RESUBMIT IT!

If you have not already done so, please print out section 6 of the PIAA form here.  This is the physical page and needs to be completed (all sections), signed and dated by an MD, DO, PAC, CRNP or SNP.  It must be dated May 1, 2024, or later to be accepted for the 2024-2025 school year.  The physical form will not be accepted with two dates on it.  Once completed it should be uploaded under the Physician section.  If you are unable to scan and submit it, the form may be submitted to the Head Athletic Trainer.  The Head Athletic Trainer will verify all registrations once submitted.  Once reviewed, including the physical, the registration will be approved, and another email notification will be sent from FormReLeaf.  If there is an issue FormReLeaf will send an email notification of the issue.  If you have not submitted a physical, you will not receive an approved notification for the registration.
