Hello Parents & Student-Athletes and Welcome to Form Releaf,

Form Releaf is a secure registration platform that provides you with an easy, user-friendly way to register for Towson High School Athletics. Spring Sports registration will be done electronically. Please scan all documents (completed physical andCOVID check off) and upload in the doctor's information portion of the registration.

To Register:
1. Select the Season, Select the Sport, Fill in all the fields, and Review Heat Acclimatization, Sudden Cardiac Awareness, Informed Consent Video, Parent/Athlete Contract, Code of Conduct, and Concussion Policies. Sign off using an Electronic Signature when you have completed the online registration process.
