High School Marching Band Registration 2026-2027

ONLINE REGISTRATION
This is the online registration form for Jefferson
Township High School. Registration for Marching Band shall be completed online
and submitted to the school before the registration deadline.
Per the State of New Jersey and the Scholastic
Student-Athlete Safety Act
With the passage of the Scholastic Student-Athlete Safety Act, New
Jersey has set the standard by requiring that thorough evaluation be conducted
for the silent cardiac abnormalities that may lead to the sudden death of a
student. The below requirements are mandated by the state of New Jersey
without exception to participate in Marching Band for the 2026-2027 school
year.
Requirements for Participation in High School Marching Band
A Freshman/Transfer/Student would be eligible for participation in Marching Band ONLY IF they meet ALL of the
following criteria:
- Parent/Guardian
and Student must complete the Maching Band Registration form via FormReleaf Registration by the deadline.
- NJ Physical Packet Forms (document)
must be used for preparticipation clearance and kept as part of your
medical records with primary care provider. NOT TO BE SHARED WITH
THE SCHOOL
- Preparticipation
Physical Evaluation Medical Eligibility Form must be
uploaded after it is signed by a health care provider who:
*Please note: ALL requirements must be met or you will
not be cleared to participate.

The 2026-2027 fee schedule is as follows:
Per Activity: $50 - Per Sport: $100
Annual Cap Per Student: $200 Annual Cap Per Family: $400
Reduced Lunch Families: 50% of all rates/caps.
Free Lunch Families: No Fee
In order for the discounts to be applied correctly, the same account log-in must be used for all registrations in the school year.
