Please print out "Health History" and "Pre-Participation Physical Evaluation (see below). Once completed by the Authorized Medical Examiner, return to the Athletic Office.
INFORMATION NEEDED TO REGISTER: It will be helpful to have the following information handy to allow for accurate completion of your online registration (example: Doctor information, Health Insurance Information, Etc...)

In addition to completing your registration, per the October 6, 2021,
BCPS Announcement, the following will be enforced during the Winter Season:
Beginning
Wednesday, November 24, 2021, BCPS will require proof of full COVID-19
vaccination for any high school student participating in BCPS winter and spring
sports for the 2021-22 school year.
Students
who are unwilling to be vaccinated, or present documentation verifying a medical
or religious exemption as to why they cannot be vaccinated, may participate in
mandatory weekly COVID-19 testing. Testing will be offered at no cost to
the student athlete at their school on a regularly scheduled basis that is
either during the school day or practice time. Tests involve a brief self-swab
inside the tip of the nose and use PCR technology to detect the COVID virus.
If a
student fails to get vaccinated or participate in the mandatory weekly COVID
testing process, they will lose their eligibility and be unable to participate
or until the requirements are met.
As details regarding the verification and/or
testing process are communicated, they will be shared with the community via
Schoology and the Lansdowne Athletics social media accounts (LansdowneAthletics
– FB, Instagram. @VIkingsLHS – Twitter). Plan accordingly.
