2022 LMS Fall Sports Registration

Hello Parents & Student Athletes and Welcome to FormReleaf!

FormReleaf is a secure registration platform that provides you with an easy, user-friendly way to register for Lansdowne Middle School Athletics. All Fall Sports registration information will be collected electronically, except for the sports physical evaluation forms.  Sports physical forms can be scanned and attached to your FormReleaf registration, or sent to the LMS main office to the attention of Ms. Young. 

To register:

1. Select the Season (Fall, Winter or Spring)

2. Fill in ALL of the fields

3. Review ALL: Heat Acclimatization, Sudden Cardiac Awareness, Informed Consent Video, Parent/Athlete Contract, Code of Conduct and Concussion Policies, LMS Student Athlete Guidelines and Agreement

4. Sign off using an Electronic Signature when you have complete the online registration process
