Fall Athletics 2022

Welcome to the Fall 2022 Season of Mercy Athletics!!!  

• Please READ all athletic forms located on this site and electronically sign BEFORE MONDAY, AUGUST 1st. Each athlete must have an online form submission and an up-to-date physical** on file with the nurse by this date, or they will be unable to participate in tryouts. This applies to ALL ATHLETES, including both upper and underclassmen. An new online form submission is required every season!

• ​NEW ATHLETES: Please click "Sign Up" to make a new account. Make sure to check that you have entered your daughter's name under the Participant Name, not a parent's name. You can check off more than one sport if your daughter is unsure at this point which sport they will be trying out for in the fall. Fill out the required information, and read and sign all the forms before submitting. You will receive a confirmation email upon submitting your registration.

• RETURNING ATHLETES: If your daughter already registered for a sport last year, simply "Login" to the website using your SAME email and password, then click on the "auto fill" drop down box and "select a past registrant". All previously saved information will populate in the required fields. Make sure to CHANGE or UPDATE any required information (e.g. phone numbers, insurance information, or medical history) as necessary. Please read and sign all new forms before submitting. You will receive a confirmation email upon submitting your registration. 

• If you did not receive a confirmation email upon completion, then your submission HAS NOT GONE THROUGH. You will need to try again. Make sure that all required fields (with red star) are filled out before submitting. Please call FormReLeaf Tech Support at (844) 367-6735 if you have any questions

• **Physicals CANNOT be uploaded on this site. Please send a copy of your daughter's physical to the main office. All athletes must have an up-to-date physical on file with the nurse PRIOR to any sport tryouts, or they will be unable to participate.
