Registration Information

All athletic registration must be completed online through FormREleaf.

• Registration is required for every athlete, every season.
• Only use one account per household.
• Once submitted, your registration will appear under "Completed Registrations" in your FormREleaf account.
• If you begin a registration and don't submit it, your progress autosaves under "Incomplete Registrations."

All updates to your registration status appear in your FormREleaf account under the Approval tab of Completed Registrations. Please check your account regularly for status changes.

All registration links and forms can be found on the MTHS Athletics Website under "More" → "Register for Sports." Late submissions will not be accepted.

Electronic Physicals

• All physicals must be uploaded online at the time you register. Paper copies are not accepted in person.
• The Preparticipation Physical Evaluation – Medical Eligibility Form is the only document that must be uploaded. All other forms may stay with your healthcare provider.

Important: If any submitted paperwork contains forged or falsified signatures, the student-athlete will not be permitted to try out for any team for the remainder of the school year.

Physical Requirements

• Physicals are valid for 365 days and must be valid on the first day of tryouts.
• The Medical Eligibility Form must be fully completed, include two provider signatures and include an office stamp.
• Incomplete forms will delay processing.
• If your child's physical expires after the submission deadline, please contact your school nurse.

Processing Timeline

Please allow several days for processing. You will receive an email at each step:

1. Registration Completed: Your registration and Medical Eligibility Form were submitted.
2. Correction Required (if applicable): Your physical is missing or incomplete, or more information is needed. Log into FormREleaf, edit your existing registration and resubmit.
3. Physical Under Review: The school nurse has reviewed your documents.
4. Final Physician Approval: The school physician has approved your documents. This is the final clearance needed to try out.

Students may NOT try out until final physician approval is received. Please follow the timeline and avoid calling the nurse's office unless necessary.

Additional Medical Forms (If Applicable)

• Asthma: Asthma Action Plan. A self-carry order from your provider is recommended so your child can keep an inhaler in their sports bag.
• Severe Allergies: Allergy Action Plan
• Diabetes: Diabetes Management Plan
• Seizures: Seizure Action Plan

Submit these directly to the school nurse after registering. Physicals for students with asthma or diabetes will not be processed until this paperwork is turned in.

Tryout Eligibility

To try out, student-athletes must have:

1. A completed registration
2. An approved Medical Eligibility Form on file
3. A final approval email from the school physician
