*Please note: For all incoming freshmen and transfer students who are not participating in a fall sport but plan to participate in a sport later in the school year, you must still complete all fields and upload your Medical Eligibility Form. Please indicate "not participating in current season" during this fall registration. We will have subsequent registration for the winter and spring seasons, at which time you will be able to register for a winter or spring team. 

ONLINE REGISTRATION

This is the online registration form for the  Morris County School of Technology. Registration for all sports shall be completed online and submitted no later than August 3, 2026 in order to be fully cleared and eligible to participate in tryouts starting August 17, 2026. Those submitting later will still be reviewed for approval and given an opportunity to join the team if cleared. 

Per the State of New Jersey and the Scholastic Student-Athlete Safety Act
With the passage of the Scholastic Student-Athlete Safety Act, New Jersey has set the standard by requiring that thorough evaluation be conducted for the silent cardiac abnormalities that may lead to the sudden death of a student-athlete. The below requirements are mandated by the state of New Jersey without exception to participate in athletics for the 2026-2027 school year.

Requirements for Participation in High School Athletics
A Freshman/Transfer/Student-Athlete would be eligible for participation in athletics ONLY IF they meet ALL of the following criteria:

1. Parent/Guardian and Student must complete the Athletic Registration form via FormReleaf Registration by the deadline.
2. Preparticipation Physical Exam Form (document) must be used for preparticipation clearance and kept as part of your medical records with primary care provider. You must upload these forms directly to Family ID during the registration process. School staff will not accept these forms in person or by mail, email, or fax. 
3. Preparticipation Physical Evaluation Medical Eligibility Form (Page 5 of the document listed above) must be uploaded after it is signed by a health care provider who:
• Student-Athlete Cardiac Assessment Professional Development Module completed and agreement signed (document)
• Sports-Related Concussion and Head Injury Fact Sheet read and agreement signed (document) 

4. If you have been diagnosed with asthma, print a copy of the Asthma Action Plan and return to the Health Office.

5. If you are required to carry an Epi Pen, print a copy of the EpiPen Form and return to the Health Office.

6. If have been diagnosed with diabetes, print a copy of the Diabetes Form and return to the Health Office.

*Please note: ALL requirements must be met you will not be cleared to participate.
