Welcome to North Country Union High School’s online sports registration page. The information requested within this online registration program is REQUIRED for any students who wish to participate in any athletic programs. You can register your student-athlete for one sport for each season.
In addition to the registration information, before any student can participate in athletic programs (including tryouts) the following requirements must be met:
1. Well Exam Form- a copy of the student’s most recent physical (well-child exam), signed by a physician, must be on file prior to the first day of practice. Physicals will expire 2 years from the date of the exam. Students with expired physicals will not be permitted to participate in practices or games until a new physical is filed with the Athletic Trainer. The well-child exam form can be found on this site and the NCUHS Athletics web page.
2. Parents/guardians are responsible for filling out the online forms completely and ensuring any and all medical information concerning their student’s health, medical concerns, injuries, medications, and allergies are reported to the Athletic Department and team coaches.
3. ALL students participating in athletics must provide proof of medical insurance (which can be provided through the online registration system). If you do not have insurance, please use the below contact information in order to obtain information on school insurance.

To begin the registration process please select the sport(s) (1 per season) and then scroll down to the "Participation Information" and fill out each field. When finished, click "Submit" located in the lower right hand corner. The program will not allow you to submit a registration unless all fields marked with an asterisk are filled in. Once your registration is complete you will receive a confirmation email.
In order to complete your student’s registration, you will need to enter all required information marked by a red asterisk, This process should take 15 minutes and you will need to have/know your student’s doctor’s contact information, your medical insurance information, etc. When you have successfully completed your student athletic registration, you will receive a confirmation email.
Fall sports- begin on Monday, August 16, 2021 for Football and Thursday, August 19, 2021 for all other fall sports.
Winter sports- begin on Monday, November 29, 2021 (all other winter sports).
Spring sports-begin on Monday, March 14, 2022 (baseball pitchers & catchers) and Monday, March 21, 2022(all spring sports).
