Spring Sports 2025

Parents/ Guardians,

If this is your first time using FormReleaf follow directions below:

FormReleaf functions best on Google Chrome

To begin it is best to create an account (keep this login information for future registrations.) When finished log into FormReleaf and go to "Find Organization" in the upper left of the screen and type in "Oxford Area High School". After "Oxford Area High School" shows up click on it, scroll down to portion that is says "Programs" where you should click on "Spring Sports 2024". Proceed to fill out the form, when completed you will need to press "Submit" in the lower right corner at the bottom of the screen. Please make sure all parts of the FormReleaf are filled out or it will not submit. When finished you should receive a confirmation email.

PHYSICALS MUST BE COMPLETED ON OR AFTER MAY 1, 2024, to be valid for the PIAA 2024-2025 athletic seasons. Once the physicals are uploaded they will be valid until April 30, 2025, or conclusion of the spring season.
