2025 Spring Athletics

Parents/Guardians,

PHYSICALS MUST BE DATED May 1, 2024, OR AFTER, to be valid for the 2024-2025 school year.

If this is the first sport you are participating in at Pennbrook:

Please

  1.  "Sign Up" by creating a profile then Log In.  
  2. Go to "Find Organization" and type in Pennbrook. 
  3. Click on "Pennbrook Middle School” then scroll down to “Programs” and click on “2025 Spring Athletics”. 
  4. Please choose a sport and scroll down to “Participant Information” and fill in the info for the student athlete for whom you are registering. 
  5. Once you have done that, “Submit” will appear at the bottom and after you click that, your student athlete will be registered for their fall sport. 

    Please 

    1. Print out Section 5: "Health History" (see below) and Section 6: "PIAA Comprehensive Initial Pre-Participation Physical Evaluation & Certificate of Authorized Medical Examiner" (see below). 
    2. Complete Section 5 and take along with Section 6 to an Authorized Medical Examiner. 
    3. Once Section 6 has been completed by the Authorized Medical Examiner, submit form directly on FormReleaf. Please contact Mrs. Pugliese in the Main Office at pugliecl@npenn.org with any questions. (Form must be completed after June 1, 2023).

    Please print to fill out and take to have the physician sign your sports physical/Re-Cert.
