PHYSICALS MUST BE DATED May 1, 2024 OR AFTER, to be valid for the 2024-2025 school year.
If this is the first sport you are participating in this school year:

"Sign Up" by creating a profile then Log In 

Go to "Find Organization" and type in Penndale 

Click on "Penndale Middle School” then scroll down to “Programs” and click on “Spring - 2025”

Please choose a sport and scroll down to “Participant Information” and fill in the info for the student athlete for whom you are registering

Once you have done that, “Submit” will appear at the bottom and after you click that, your student athlete will be registered for their spring sport

You will receive an email to confirm registration. If you do not receive a confirmation email, your student is not registered and will not be able to participate until this is complete.

Print out Section 5: "Health History" (see below) and Section 6: "PIAA Comprehensive Initial Pre-Participation Physical Evaluation & Certificate of Authorized Medical Examiner" (see below). 

Complete Section 5 and take along with Section 6 to an Authorized Medical Examiner. 

Once Section 6 has been completed by the Authorized Medical Examiner, you must upload the form to FormReleaf when prompted.  (Form must be signed after May 1, 2024)

Please print to fill out and take to have the physician sign your sports physical/Re-Cert.

If you have previously registered an athlete on FormREleaf.com:

• Please “Login” using your previous login information. “Click on Penndale Middle School” and scroll down to “Programs” and click on the specific sports season.

• Please choose a sport and scroll down to “Participant Information” and select “Auto Fill” for the student athlete for whom you are registering. The data will be filled in (this is the time to update any changes you may have) and you MUST re-sign each item at the bottom, which will recertify your child for that season.

• Once you have done that, “Submit” will appear at the bottom at the bottom of the screen. Please hit submit and your student will be registered for their sport. 

• You will receive a confirmation email.
