Hello Parents and Student-Athletes

Welcome to Form Releaf!

Form Releaf is a secure registration platform that provides you with an easy, user-friendly way to register for Pine Grove Middle School Athletics. Fall Sports registration will be done electronically, including uploading the current physical.

To Register: 
1. Select the Season, Select the Sport, Fill in all the fields, and Review Heat Acclimatization, Sudden Cardiac Awareness, Informed Consent Video, Parent/Athlete Contract, Code of Conduct, and Concussion Policies. Sign off using an Electronic Signature when you have completed the online registration process.  

2. All students MUST have a current physical on file.  A physical is valid for 14 months , so please make sure your athlete is current with their physicals to maintain eligibility. 

This is the Physical Form that must be completed by a physical with a signature and/or stamp.  In addition, this must be uploaded to your form (either scanned or with pictures taken of the entire form).  If you have a current physical on file, please upload a picture or document indicating, and I will check.
