Sayreville Fall Athletics 2026

NEW SYSTEM!!!!  MUST CREATE NEW LOGIN for 2026-2027!!!

This is the online registration form (PART 1) for Sayreville Public Schools. Registration for all sports shall be completed online and submitted to the school before the registration deadline.

To be fully cleared for each season, please complete steps 2 and 3 as well.

Part 2:
Sayreville Health History
(Must be completed for every sport, each season)
(Please give a Hard Copy to the nurses office during school  - ***during the summer all high school and middle school forms get dropped off to the High School: Drop Box Located at the Main Entrance)

Medical Eligibility Form -NEW* (Must be given to Doctor with Pre-Participation Physical Form)                                                            The Doctor will keep the Pre-Participation Physical Form                                                                                   *****However you MUST return the Medical Eligibility Form - One Page - Signed,  Stamped, and Signed again under the “Student-Athlete Cardiac Assessment Professional Development Module”        (Please give a Hard Copy to the nurses office during school  - ***during the summer all high school and middle school forms get dropped off to the High School Drop Box Located at the Main Entrance)

Pre-Participation Physical Forms - ***Print For Doctor use only 
Please print and give this form to your child's doctor at the time of their yearly physical - This form  stays with your Doctor and DOES NOT have to return be returned to the school.

The only forms that MUST be returned to the nurse are the Sayreville Health History form (every season) and the  Medical Eligibility Form (Yearly)

**Current Physical Means**
Physical examinations are only good for 364 days.

Part 3:
Impact Test Procedures
High school student Only Take the Impact Test once a YEAR after July 1st of the upcoming school year
Middle School students only have to take the Impact Test ONCE after they have tried out of a team. Middle School Students ONLY have to take Impact test ONCE for their entire Middle School Career. Coaches will provide more instructions.
