2018-2019 Athletics

Parents/Guardians & Students:

We would like to welcome all new and returning Eagles for the 2018-2019 school year! We are very proud of our athletic program and happy that you are interested in becoming part of it. Filling out this online form is just the first step. 

Medical forms still need to be completed by a primary care provider or at the School Based Health Center (575-627-2808) and returned to your coach at Sierra. You can download and print these forms: CLICK HERE (ENGLISH) OR CLICK HERE (SPANISH). Printed copies are also available in the main office. Physical exams completed on or after April 1, 2018 are considered valid through the following school year. A student must have a current physical on file for any participation outside of the school day. (per NMAA Eligibility By-Laws 6.15)

Student athletes are required to have medical insurance to participate in practices and games.  If you do not have insurance you can purchase school insurance from www.myers-stevens.com. Proof of purchase will be required to be eligible to participate.  Once you receive your medical ID card we will need a copy to keep on file. 

Once you have completed the online forms and returned the completed medical history and physical exams forms you will receive an approval email indicating you are eligible to participate.
