South Hunterdon Regional School District Athletic Registration

*Please note: For all incoming freshmen and transfer students who are not participating in a fall sport but plan to participate in a sport later in the school year, you must still complete all fields and upload your Medical Eligibility Form. Please indicate "not participating in athletics" during this fall registration. We will have subsequent registration for the winter and spring seasons, at which time you will be able to register for a winter or spring team. 

ONLINE REGISTRATION

Welcome to the online registration form for the South Hunterdon Regional School District. Registration for all sports shall be completed online and submitted to the school before the registration deadline.

In accordance with the Scholastic Student-Athlete Safety Act, New Jersey has set the standard by requiring that a thorough evaluation be conducted for the silent cardiac abnormalities that may lead to the sudden death of a student-athlete. The below requirements are mandated by the state of New Jersey without exception to participate in athletics for the 2026-2027 school year.

Requirements for Participation in High School Athletics
A Freshman/Transfer/Student-Athlete would be eligible for participation in athletics ONLY IF they meet ALL of the following criteria:

1. Parent/Guardian and Student must complete the Athletic Registration Form via FormReleaf Registration by the deadline.
2. Preparticipation Physical Exam Form in English / Spanish must be used for preparticipation clearance and kept as part of your medical records with your primary care provider. Only the last page should be uploaded and shared with the school.
3. The Health History Update Questionnaire Form: Is required if your physical is more than 90 days old from the first day of participation for each season in English / Spanish. 
4. Review and acknowledge you have read all required policies, information, and agreements in the online registration form.
5. Send in the Student Activity Form in English / Spanish with Payment to the Main Office.
6. You can now pay your activity fee online by visiting payschoolscentral.com.  On the home screen you will see "meals" where you normally add cafeteria funds and a "fee" box right underneath it where they will pay the Activity Fee.  Click the cart to add/pay the fee.

*Please note: ALL requirements must be met you will not be cleared to participate.
