STEP 1:
All students who plan to participate in athletics must have a physical
examination on file within 365 days of the start of the season. All physicals
must be approved by our school physician prior to the start of participation.
Athletes will not be allowed to participate without having a physical exam in
good standing.

Physical Evaluation Medical Eligibility Form / Evaluación
Física Previa a la Participación Formulario de Historical Clínico to
the school.
- If applicable, please use this Athletes
with Disabilities Form supplement to the Preparticipation
Physical Evaluation Medical History Form. This form should not be
returned to school.
STEP 2:
Complete online concussion
baseline testing. You will receive an e-mail from Mrs. Somma, via your
school email account, if you are due to take a concussion baseline test.
Baseline testing is required every other year and must be completed prior
to participation. If you have any questions or concerns regarding
baseline testing, please contact Mrs. Somma.
STEP 3:
Please review and complete this Student
Activity Fee form and submit payment to the main office.
STEP 4:
Athletes are not considered cleared for participation until parents receive an
email from Mrs. Somma providing clearance for their participation.
STEP 5:
If you already have a current physical on file in the school's Health Office
but the start of the athletic season is more than 90 days after the date of
your most recent physical, you will need to fill out the Health
History Update Form / Cuestionario
de Actualización del Historial Médico and return it to the School
Nurse and/or Athletic Trainer.
