2021-2022 Winter Athletics
Parents/Guardians,
BEFORE YOU REGISTER:
Physicals are valid for one year from the physical exam date. Ex. If your physical exam date is 2/1/16, it expires on 2/1/17. It is strongly suggested that all athletes get a new physical after June 1, 2018. Any physical after that date will be good for all three sports seasons of the 2018-19 school year.
Please print out "Health History" and "Pre-Participation Physical Evaluation (see below). Once completed by the Authorized Medical Examiner, return both the "Physical" page and the "History" page to Mr. Latanishen. All forms must be stamped by the Doctor's office near the Doctor's signature line.
INFORMATION NEEDED TO REGISTER: Before going online to register, It will be helpful to have the following information handy to allow for accurate completion of your online registration (example: Doctor Information, Health Insurance Information, Etc...)

Please print to fill out and bring to a doctor to have the physician sign your sports physical.

In addition to completing your registration, per the October 6, 2021 BCPS Announcement, the following will be enforced during the Winter Season:
Beginning Wednesday, November 24, 2021, BCPS will require proof of full COVID-19 vaccination for any high school student participating in BCPS winter and spring sports for the 2021-22 school year.
Students who are unwilling to be vaccinated, or present documentation verifying a medical or religious exemption as to why they cannot be vaccinated, may participate in mandatory weekly COVID-19 testing. Testing will be offered at no cost to the student athlete at their school on a regularly scheduled basis that is either during the school day or practice time. Tests involve a brief self-swab inside the tip of the nose and use PCR technology to detect the COVID virus.
If a student fails to get vaccinated or participate in the mandatory weekly COVID testing process, they will lose their eligibility and be unable to participate or until the requirements are met.
As details regarding the verification and/or testing process are communicated, they will be shared with the community via Schoology
