Parents/Guardians,

BEFORE YOU REGISTER:

INFORMATION NEEDED TO REGISTER: It will be helpful to have the following information handy to allow for accurate completion of your online registration (example: Doctor information, Health Insurance Information, Etc...)

FREE Sports Physicals by Appointment with Choptank Community Health!! Choptank Enrollment

*will also have appointments available at Easton Elementary on Mondays and Wednesdays

REMINDER: You MUST bring your completed and signed sports physical paperwork/forms with you to your appointment.

Sports Physicals can be found in the EHS Main office or by emailing Ms. Hoffman, khoffman@talbotschools.org 

Once your physical is complete, please upload ALL PAGES to the platform, if you are unable to do so please email it to the athletic trainer/athletic director. 

All forms must be completed before an athlete will be allowed to participate in tryouts or out of season workout sessions. (physicals are good for one calendar year so if you had a physical completed last year for the winter or spring season your physical is still good.)

If you have any questions about tryout times, or you were not able to make the orientation please contact your athletic director for more information.

9 de julio en EHS y 10 de julio en SMES

*También habrá citas disponibles en Easton Elementary los lunes y miércoles.

RECORDATORIO: DEBE traer a su cita de  examen físico los documentos/formularios requeridos, ellos deberían ser completos y firmados.

Los formularios requeridos para los exámenes físicos deportivos se pueden encontrar en la oficina principal de EHS o enviando un correo electrónico a la Sra. Hoffman, khoffman@talbotschools.org 

Una vez que se complete su examen físico, cargue TODAS LAS PÁGINAS en la plataforma; si no puede hacerlo, envíelo por correo electrónico al entrenador deportivo/director deportivo. 

Todos los formularios deben completarse antes de que a un atleta se le permita participar en pruebas o sesiones de entrenamiento fuera de temporada. (Los exámenes físicos son válidos por un año,  sí se realizó un examen físico el año pasado para la temporada de invierno o primavera, su examen físico aún es bueno)

Si tiene alguna pregunta sobre los horarios de las pruebas o no pudo asistir a la orientación, comuníquese con su director deportivo para obtener más información.
