2018-2019 Sports

PARENTS: Thank you for using our online sports registration site. The following three items must be completed and on file before your child will be allowed to participate in athletics.

Please follow these instructions:

• Click the links below to PRINT and COMPLETE (1) Physical form, and (2) Concussion form.

• Please note that you must print and turn in the Physical Form and Concussion Form in paper format. Forms must be returned to Athletic Director Deuce Harris via the Voyager Academy High School Front Office.

• The Physical form is valid for one year from date of physical exam. The Concussion form is valid for one year from date of completion.

• (3) Next, at the bottom of this page are all of the sports outlined below in the box called "sections" where you will choose your player's sport(s). HS=High School. MS=Middle School. 

• Click the appropriate sport(s), and fill in all relevant questions (any question with a red star is required to complete registration).

• YOU MAY SELECT MORE THAN ONE SPORT, if applicable.

• Read all online forms and be sure to electronically sign all areas where indicated.

• Click “Submit” and verify that you have completed registration. You will receive an email message stating that online registration is complete.

• The Athletic Department cannot see partially completed online registration. Please contact technical support if you have issues completing and submitting online forms.

Athletic Fees:

Please remember that once a child is selected for an athletic team, athletic fees are due before that child may participate in any games or matches. Speak with Athletic Director Harris to discuss payment plans or if you have any questions or concerns about paying the athletic fee.

Middle School: First sport $105; each additional sport is$75 each

High School: First Sport $135; each additional sport is $95 each 

*Reduced fee available for those that qualify for free or reduced school lunch
