2020-2021 Sports

PARENTS: Thank you for using our online sports registration site. The following four items must be completed and on file before your child will be allowed to participate in athletics.

  1. Physical Form (Must be uploaded)

    1. If you have a valid physical on file already from past seasons, you still must complete and upload page 1 of the physical form. 

  2. Concussion Form (Must be uploaded)

  3. NCHSAA Eligibility, Consent to Participate and Release Form  (Must be uploaded)

    a. Includes watching "Crash Course" concussion video

  4. Covid-19 Screening Form (Must be uploaded)


Please follow these instructions:

  1. At the bottom of this page are all of the sports outlined below in the box called "sections" where you will choose your player's sport(s). 

  2. Click the appropriate sport(s), and fill in all relevant questions (any question with a red star is required to complete registration).

  3. YOU MAY SELECT MORE THAN ONE SPORT per registration, if applicable.

  4. Please upload all forms in the Participant Section. Take a photo or scan a copy of the form. Upload these files to the Form Releaf account. Save the paper copies in a safe place at home. Contact the Athletic Director if you are unable to upload to Form Releaf.

  5. The Physical form is valid if it is dated on or after 03/01/2019. See the chart below:

Requirement for 2020-2021

Prior to 03/01/19

Need valid, up-to-date physical prior to participating.

On or after 03/01/19 through 05/01/20

Granted a temporary extension to participate through the end of the 2020-2021 academic year.

On or after 05/02/20

Will participate based on the current policy of being “valid for 395 days”


  1. Read all forms and electronically sign all areas where indicated.

  2. Click “Submit” and verify that you have completed registration. You will receive an email message stating that online registration is complete.

  3. The Athletic Department cannot see partially completed online registration. Please contact Form Releaf technical support if you have issues completing and submitting online forms.
