Fall Athletic Registration 2026

ONLINE REGISTRATION

This is the online registration form for Wayne Hills High School. Registration for all sports shall be completed online and the Preparticipation Physical Evaluation Medical Eligibility Form must be submitted to the school before the registration deadline. YOU MUST BE REGISTERED BEFORE HANDING ANY MEDICAL FORMS INTO THE NURSE.

Requirements for Participation in High School Athletics

A Freshman/Transfer/Student-Athlete would be eligible for participation in athletics ONLY IF they meet ALL of the following criteria:

1. Parent/Guardian and Student must complete the Athletic Registration form via Form RELeaf Registration by the deadline.
2. Preparticipation Physical Exam Form (CLICK HERE) must be used for preparticipation clearance and kept as part of your medical records with your primary care provider. You are only required to turn in the Preparticipation Physical Evaluation Medical Eligibility Form to the Nurse. 
3. The HEALTH HISTORY UPDATE should be completed and uploaded in Form RELeaf in the Participant Information Section every season (even if you are turning in a physical).
