All students must be registered for EACH SEASON on the Athletics Registration site found on waynevalleyathletics.com

Download the Pre-Participation Physical Evaluation and History Form (to be completed by a physician) HERE.

Print and physically submit the completed form to the school nurse.

First Official Practice Dates

8/10 - Football, Tennis

8/17 - Cross Country, Field Hockey, Gymnastics, Soccer, Volleyball

NOTE:  Athletic physicals are current for 365 days from the date of exam, not the date the paperwork was signed by the doctor.  

IMPORTANT REMINDER:  Students who are excluded from PE participation for any reason are not permitted to participate in athletics.
