Hello Parents & Student-Athletes and Welcome to Form Releaf, 

Form Releaf is a secure registration platform that provides you with an easy, user-friendly way to register for BCPS Athletics. Sports registration will be done electronically except for the sports physical forms and the Covid-19 sign-off sheet. 

To Register: 

1. Fill in all the fields, and Review Heat Acclimatization, Sudden Cardiac Awareness, Informed Consent Video, Parent/Athlete Contract, Code of Conduct, and Concussion Policies. Sign off using an Electronic Signature when you have completed the online registration process.  

2. Please print, take to a doctor, and then upload the completed Physical Form with the Covid-19 Update-mpssaa.org

3. Bring a hard copy of your physical to Mr. Berkowitz
