2020-21 Athletic Clearance Registration

ATTENTION:

Prior to beginning the clearance process, you must have the following information completed in digital form:

  1. ​An FHSAA EL2 - Preparticipation Physical Form Signed and Dated by all Parties, including a Licensed Physician
  2. Copy of Insurance Card -OR- a CCA student accident insurance application (additional cost)

Student insurance is required. If you do not have coverage for your student-athlete, you can purchase a supplemental policy through the school's accident insurance. They are available at the link in part 4 above.

PLEASE DO NOT PROCEED WITHOUT THESE DOCUMENTS COMPLETED AND SAVED IN DIGITAL FORM
