Spring 2024 Athletics

To Canisius Student-Athletes and their Families;

In an attempt to streamline our paperwork process prior to tryouts, we have partnered with Form ReLeaf. Each student looking to play a sport must register in this system.

Once you have registered, you will become a part of the system. For example, if you play a Fall sport and want to play another in the Spring, you will only need to update your info at that time. This takes the place of the Health Update form.

All Canisius students still must have an up-to-date Physical on file in the Nurse's Office. 

Here are the two items a student must have prior to tryouts...

1 - Register in the Form ReLeaf system.

2 - Provide the School Nurse an up-to-date Physical.
