2024-25 SPRING only Governor Livingston High School Athletic Registration

We are now working on Registrations for SPRING Sports.

Step 1 Online Registration (via Form Releaf)

Step 2 (A and B) Sports Physical and Health History Update Forms (all sent to glsportsregistration@bhpsnj.org)

Step 3 Participation Fee (via Payschools Central or via Check) (Instructions can be found HERE)

Step 1: Complete the online sports registration on Form ReLeaf:

Online Registration through FormReLeaf will open for the 2024 Spring Season on Monday January 13th, 2025. Please complete the online registration ASAP (preferably by February 3rd). The Health Office and Athletic Trainer's Office(s) will continue to collect paperwork via glsportsregistration@bhpsnj.org, as necessary.

• Full time Union Magnet/ Vo-Tech Students ARE able to register on Form Releaf.

Step 2A: Sports Physical (Once per calendar year)

Physical date must be within 364 days of the START of the respective season. Unsure if you need to submit a new one at this time? Email glsportsregistration@bhpsnj.org to inquire.

Please note ALL physicals completed by your physician must be approved by our School Physician. This process can take 4-5 days once we have received it.  The sooner we receive it, the better.  If there are issues with appointment dates with your insurance contact us at glsportsregistration@bhpsnj.org for guidance.

If you are unable to get an appointment at your Doctor's Office.  Call our School District Doctor, Care Station (Springfield Office, 90 US Route 22) at (908) 925-2273, for an appointment (at no cost to you).  Let them know that you are a Governor Livingston High School Student-Athlete and bring the 3 Forms below to be completed by the Doctor.  

1.  Health History Form to be signed and stored at your Health Care Provider/Doctor’s Office (NOT THE HIGH SCHOOL):

3. Preparticipation Physical Evaluation Medical Eligibility Form to be completed and signed by your Health Care Provider /Doctor’s Office. Please make sure they sign ALL REQUIRED AREAS

THIS FORM MUST BE HANDED INTO THE HIGH SCHOOL FOR CLEARANCE: Scan the completed Preparticipation Physical Evaluation Medical Eligibility form and email the signed completed form to GLSportsregistration@bhpsnj.org

• All public school districts are required to provide to the parent/guardian written notification signed by the school physician indicating that the Medical Eligibility Form has been received, reviewed by the School Physician, and that the student athlete has either been deemed medically eligible ("cleared") or not medically eligible ("not cleared") as per recommendations of the Health Care Provider completing the evaluation and Medical Eligibility Forms.  In compliance with this requirement, an email from Form ReLeaf will be sent once the student athlete is cleared or not cleared.

MEDICATION FORMS:

If the student-athlete requires emergency medication (EpiPen or inhaler), forms including permission for self-administration, completed by the private physician are required.  Any student requiring medication must submit the appropriate paperwork to the Health Office prior to the start of preseason.
