Parents/Guardians,

BEFORE YOU REGISTER: No athlete may participate in practice or games until a valid physical evaluation is submitted and online registration approved. The "Form ReLeaf" (online registration) and "Pre-Participation Physical Evaluation" are linked below. Once a parent/guardian submits completed documentation through Form ReLeaf and the student submits a completed physical form to their coach, items will be reviewed for clearance to play. Physicals are valid for 13 months from the date documented on the form.

INFORMATION NEEDED TO REGISTER: It will be helpful to have the following information handy to allow for accurate completion of your online registration (example:Doctor information, Health Insurance Information, Etc...)

REGISTRATION PHYSICAL PROCESS:

1. A parent/guardian should register on Form ReLeaf, which includes acceptance of information and risks associated with sport, insurance submission, emergency contact information and Overlea Athletics behavior policy, expectations, and possible consequences. CLICK HERE TO ENTER FORM RELEAF PORTAL

2.  Print to fill out the physical form for a doctor complete and sign your sports physical. It must state that the athlete is cleared for athletics. CLICK HERE FOR PHYSICAL FORM.

 Physical forms may be dropped off in the main office to the attention of Mrs. Luby.
