BEFORE YOU REGISTER:

Physicals are good for 14 months.

Print out the physical form for your doctor to fill out then return it to your coach.

INFORMATION NEEDED TO REGISTER: 

• Medical subscriber's name (parent/guardian)
• Insurance company/provider's name
• Insurance group number
• Parent email and mobile phone number
• Emergency contact information (2 people)
• Athlete grade, date of birth, address
• Click on your sport then enter all information.
