2021-2022 Winter Sports ( Basketball).

Hello Parents & Student-Athletes and Welcome to Ridgely Middle Sports , 


We will be using FormReleaf.com to register for all BCPS sponsored sports


Form Releaf is a secure registration platform that provides you with an easy, user-friendly way to register for Ridgely Middle School Athletics. Winter  Sports registration will be done electronically including the sports physical forms and the Covid-19 sign-off sheet. Sports physical forms can be scanned uploaded  to this site. Physician must stamp the physical. 

To Register: 
1. Select the Season, Select the Sport, Fill in all the required fields, and Review Heat Acclimatization, Sudden Cardiac Awareness, Informed Consent Video, Parent/Athlete Contract, Code of Conduct, and Concussion Policies. Sign off using an Electronic Signature when you have completed the online registration process.  

2. Upload your child's Physical Form with the Covid-19 Update- Required Sports Physical with Covid questions

3. Basketball tryouts are required to make the team. There are only two teams (boys and girls)
