SRDS Fall Athletics Registration Instructions
Welcome to Saddle River Day School's online athletic registration system.

To participate in SRDS Athletics, students must be fully registered and medically cleared in accordance with the New Jersey State Interscholastic Athletic Association (NJSIAA) and the New Jersey Department of Education (NJDOE) guidelines.

Pre-Participation Physical Evaluation (PPE) — Required

As mandated by the Scholastic-Athlete Safety Act (2015-2016):
A licensed physician, advanced practice nurse (APN), or physician assistant (PA) who has completed the Student-Athlete Cardiac Assessment professional development module must conduct the physical.

You are encouraged to confirm your provider has completed this module before scheduling an exam.

Submitting the PPE

• A New Jersey Pre-Participation Physical Evaluation (PPE) Form must be:

  • Partially completed and signed by the parent/guardian (History Form)

  • Completed, signed, and stamped by a licensed medical provider

• PPE is valid for 365 days from the date of the exam (For Fall sports approval all PPE must be dated on or after 8/17/2025)

• The school nurse must review and approve all forms before any participation

• You may:

  • Upload a scanned image of the PPE in this online registration

  • OR submit a hard copy/email a copy to the school nurse's office

Important Notes

• If a physical expires mid-season, the athlete must stop participation until a new physical is submitted.

• Registration is not complete until:

  • PPE Form is submitted and approved

  • Seasonal health update and consent forms are completed

• All documents must be received and reviewed for a student to be eligible
